CDのルートは/ cdromなので、ルートのpreseed.cfgは次のようにする必要があります。

file=/cdrom/preseed.cfg

もちろん、preseedのフォルダーを作成してパスを変更できます（例：フォルダー/ responsesはfile =/cdrom/responses/preseed.cfgになります）
